The leak is almost never the CPM. It is the WhatsApp inbox that cannot book, the Arabic page that does not exist, and the weekly report that cannot name an appointment.

A Doha clinic can spend well on Meta and still have a quiet calendar. The ad did its job: a patient tapped through and sent a message. Then the message sat. After hours, a competing clinic answered first. That is not a creative problem.

A clinic growth system is the closed path from that tap to a confirmed slot. It needs a bilingual page, an inbox that can offer times, a calendar the doctors actually honour, and a report that attributes the appointment to the campaign — not to ‘WhatsApp’ as a blob.
